The Salvation Army will take applications for the Angel Tree 2016 Christmas Assistance program in early October.
This program provides assistance for families with children up to 12 years of age in Kay County only.
Parents can apply for the program from 5 to 8 p.m. Oct. 10-14 at the new Salvation Army office at 1804 North Fifth Street in Ponca City. The phone number is (580) 762-7501.
When coming on the application dates, parents are asked to bring:
- A current photo ID (a driver's license or State ID);
- A proof of residence (utility bill, phone bill or rental agreement);
- Mail that matches your ID with ZIP codes;
- Proof of household income (a paycheck stub, DHS paperwork, unemployment documentation, food stamps documentation or any other documenation that shows how you maintain your household.)
- Proof of Children: (TANF papers; birth certificate; shot records that show date of birth; any legal form that has your name and the child’s name and date of birth). Social Security cards will NOT be accepted as proof that this is your child.
Applications are accepted on a first come, first served basis. Families seeking help from The Salvation Army are not eligible for Christmas assistance from any other assistance agency.
Please know the clothing and shoe sizes, three toys, three needs, and reading level of your children before arrival.
DO NOT BRING ANY CHILDREN WITH YOU OR WE WILL NOT BE ABLE TO REGISTER YOUR CHILDREN; NO EXCEPTIONS.
